Please see attached Work Plan for details. A mandatory pre-bid meeting will be held on Monday, June 27,2022 at 9am. Please meet at the first address listed above. If your firm/company is awarded the bid, the following items must be submitted within three business days in order to issue a Purchase Order:
• Proof of Florida Asbestos License
• Current W9 (less than one year old)
• Worker’s Compensation as required by State Law ($1,000,000 coverage) OR Exemption Form
• General Liability ($1,000,000 coverage)
• Automobile Liability ($1,000,000 coverage)
• Certificate of Insurance (COI) must list Escambia County as Certificate Holder and additional insured for General and Auto Liability (see example COI)
