Response: There is no specific bid form for a RFQ. The department has asked for lump pricing as noted in the scope of work attached. All bids are to be submitted via Vendor Registry.

Question 4

Posted: 11/30/2021

Question: Martyn, Good afternoon. I went out to the project this morning and I have the below questions regarding this RFQ. 1. The concrete and asphalt that is removed to install the pipe system, is it to be hauled away to an approved dump location or can we stockpile it on-site adjacent to the new pipe system? 2. The power pole at the edge of the fence will need to be relocated before work can be done for safety reasons. Who will be responsible for this? 3. Should rock be encountered during the pipe system excavation, how will this be handled? By a change order if it is encountered? 4. Should the existing asphalt and concrete to be removed be excessively thick over 6in, we would need to rent a hammer to remove it. Since we do not know the thickness, should we assume 6in and if it is thicker, a change order will be issued to cover the extra expense for removal. 5. Will the City or the contractor need to do the construction survey layout staking and at who's expense? 6. Will stone bedding be required under and on the sides of the pipe for the entire new pipe system? 7. Can the contractor get access to the project thru the entrance to Neese towing? 8. Is the bid to be submitted via email to you or hand delivered to you at what address? 9. Will a bid bond, performance and payment bond be required? Thank you, Scott A. Beaumont Chatfield Contracting, Inc.
