Question: On page 17 of the RFP it states that the following items are not applicable Item 2 - Proposal Security Item 3 - Notarized Declaration letter from Surety However in the Contractors Qualifications, these items are being asked for. Can you please clarify if we need to provide the Bond and the letter. Thank you!

Question: 1. Will a wage decision be provided? 2. Price Proposal Form-There is no forms with this as a header. Can you confirm what forms should be considered part of the price proposal form? 3. Is the price points calculated off of the LABOR RATES RESPONSE FORMs only? 4. Does the fixed fee % or the Equipment table factor into the price points calculation? 5. Does the fixed fee % apply to materials used in projects? 6. Can a sample contract be provided?

Question: 1. On the Labor Rates Response Form for (Projects Under $60,000.00) Between Position #13 and Position #14 is a line labeled as "Labor Categories" is this supposed to a be a labor category? On Labor Rates Response Form for (Projects Over $60,000.00) between Position #7 and #8 also has a line labeled "Labor Categories". Should this be another labor category? 2. Laborer Group III and Cable Splicer Position #5 are two different categories in typical wage decisions. Should these be separated? 3. Laborers in typical wage decisions go up to Group IV, the labor forms have the Laborer Groups going up to VI. What should be Laborer Group V Position # 6 and Laborer Group VI Position #7?

Question: Section 2.4 states that we must provide discount and list price on a material list but, the fixed fee table says that we are to include parts and materials to calculate the fixed fee percentage. Are we using a list price plus a discount to calculate a price and then attaching a fixed fee based on that or is the fixed fee based on actual cost?
