Description: PROJECT: Replacement of Portion of Distressed Concrete Slab LOCATION: 723 Ohio Avenue, Etowah Carnegie Library The City of Etowah, TN is currently soliciting bids for the rehabilitation of concrete pavement located at the Etowah Carnegie Library between the Library building and the Fire Hall. The scope of the project is as follows: 1) All new work will be compliant in every regard with the Americans with Disabilities Act 2) The contractor is responsible for the removal of existing asphalt and concrete from the curb of Ohio Avenue approximately 14 feet into the paved area between the Library and Fire Hall 3) The contractor will install new base material, no less than 4” thick and conforming to all applicable International Building Code standards. Underlying soil will be compacted if necessary. 4) Replacement concrete must be reinforced and a minimum of 3,500 psi 5) The new slab must be a minimum of 4” thick 6) Contractor will frame, pour and brush-finish the new pavement. 7) The contractor will ensure that new pavement is sloped to drain storm water towards Ohio Avenue 8) Workers must observe Municipal Ordinances regarding noise and construction activities. The noise curfew is from 7:00 a.m. to 11:00 p.m. However, work should cease at dusk. For review of this project, this site will be available on Tuesday, May 16, 2017 from 11:30 am until 12:30 PM. Sealed Bids should be submitted to Etowah City Hall (723 Ohio Avenue, Etowah TN 37331) no later than the close of Business on Friday May 26. 2017. All Bids must contain the following information: A. The itemized cost of the project including. o Each aspect of the project listed separately. o Proposed products to be used in addition to product specifications. B. Proof of Required Insurance o Commercial General Liability – $1,000,000.00 o Worker’s Compensation – statutory limit C. Copy of valid Business License and Full Contact information. D.
