NOTICE OF REQUEST FOR BIDS
COVID-19 SUPPLIES
KFD21-005
The City of
Kingman is soliciting bids and specifications for the City of Kingman Fire
Department Covid-19 supplies as specified in Appendix A of this advertisement. Bid
price is to include shipping costs and delivery. All vendors must be able to
meet the requirements stipulated by the Federal Emergency Management Agency
(FEMA) as well as the procurements policies of the city of Kingman.
Please submit
written / sealed bid proposal to the office of the City Clerk located at 310 North 4th Street, Kingman, AZ.
86401. Bids will be accepted until 3:00
pm local time on November 17, 2021.
Bids will be opened and announced at this time. Any proposal received after 3:00 P.M.
on the above stated date will be returned unopened. The outside of the proposal
envelope shall indicate the name and address of the proposer, shall be
addressed to City Clerk, City of Kingman, at the above address and shall be
marked: “SEALED BID – “Kingman Fire Department Covid-19 Supplies KFD21-005” and
shall contain one (1) original and three (3) copies of the bid proposal.
During the
evaluation process, the City of Kingman reserves the right, where it may serve
the City’s best interest, to request additional information or clarifications
from proposers. At the discretion of the City of Kingman, firms submitting
proposals may be requested to make oral presentations as part of the evaluation
process.
The City of
Kingman reserves the right to retain all proposals submitted and to use any
ideas in a proposal regardless of whether that proposal is selected. Submission
of a proposal indicates acceptance by the firm of the conditions contained in
this bid proposal, unless clearly and specifically noted in the proposal
submitted, and confirmed in the contract between the City of Kingman and the
firm selected.
The City of
Kingman may cancel the solicitation or reject any and all offers when the
cancellation or rejection is in the best interest of the of the City.
If you have
any questions regarding the specifications for the Covid-19 supply items listed
in Appendix A, please contact Jack Yeager, Assistant Fire Chief at jyeager@cityofkingman.gov or (928) 753-2891. In the
event that a vendor is unable to provide the specified item or an equivalent,
the vendor shall state “unable to attain.” Any additional information or

APPENDIX A.
BID SHEET
The City of
Kingman is soliciting bids for the Kingman Fire Department Assistance to
Firefighters Grant Covid-19 supplies. This bid proposal must meet all of the
bid specifications and requirements. If,
after the bid is awarded, the amounts specified in Appendix A are adjusted, the
Kingman Fire Department will contact the vendor and a replacement item quantity
will be agreed upon. The bid price must
be total delivery price to include shipping costs as well to City of Kingman
Fire Department 412 East Oak Street, Kingman, AZ. 86401.
