Overview: Q:  Do the attachments in response to the questions listed on the Statement of Qualifications count towards the 25 maximum page count?  

A:  Yes  

Q:  Can the page count be extended due to the request to list all Current and Completed Projects?  
A:  No, please submit a supplemental proposal for any additional completed projects that cannot fit into the 25-page limit.  

Q:  Are only two tab dividers required for Experience & Capacity & References?  
A:  Yes

Q:  Which tabbed section do the Contact Information and Statement of Qualifications forms go in?
A:  The Experience and Capacity section of your proposal.

Type of Addition: Addendum No. 2 (Q & A)

Overview: Q:  On the Statement of Qualifications, Required Attachment 4, they are asking for an audited balance sheet for the last 3 years. We do not do audited balance sheets annually and the majority of contractor’s do not. Can we submit either a CPA Reviewed Financial Statement for the past 3 years or a Letter from our Surety Stating our bonding capacity and single project limit in lieu of this?

A:  Yes, either a CPA Reviewed Financial Statement or a Letter from the Surety confirming Bonding capacity and single project limit would be an acceptable alternative to the Audited Balance Sheet.

Q:  On the Statement of Qualifications, Required Attachment 7, they are asking for a required safety program. Do we need do submit our Employee Safety Handbook and Policies?
A:  Proof (by link to an electronic document) in the SOQ would be acceptable. We do not want the entire Employee Safety Handbook and Policies copied and included in the SOQ. We just want to be able to confirm that you have one in place.

Q:  Are submissions due by 5:00 pm on Tuesday, October 24th? I did not see a time, only a date. 
A:  The submissions are due by 10:00 a.m. on October 24th.
