Description: The City of Lebanon, Tennessee (City), an Equal Opportunity, Affirmative Action Employer, invites interested and qualified consulting firms to submit a qualifications- based proposal responsive to the issues and requirements outlined in this Request for Qualifications (RFQ) for playground resurfacing and ADA improvements to Don Fox Community Park.

Required Scope of Services:
1. Review the existing park location and utility services.
2. Prepare conceptual drawings for playground poured-in-place surfacing and ADA parking/sidewalk improvements.
3. Prepare plans to modify utilities as necessary.
4. Prepare final plans and specifications acceptable for public bids.
5. Assist with bid advertisement.
6. Assist with review of bids and qualifications of contractor(s.)
7. Provide onsite inspection services during construction, as requested by the City.
8. Assist in completing grant management tasks through the TDEC Grant Management System.
