Response: MBE/WBE form are part of the bid packet (pages 20-24).

Question: 1) Is there a water source available on site for Contractor? Hydrant or hose bib? Where are they located? 2) The Aluminum C Channel CS 6x6.42 doesn't seem to be available given it is a Canadian Standard Channel. Do you have an alternate or a supplier?
Response: If you plan on using City water, note there is a $3,398 deposit for the hydrant meter and water is billed out at approximately $5.75/ 1,000 gallons.

Question: The detail provided for the Guard At Pipe Outfalls only provides details for pipe up 42". There is one 60" pipe out falls. Can details or direction be provided for the 60" outfall pipe guard?
Response: The guards are available for 60” pipe as well. Shop drawing approval will b required prior to ordering. Environmental Composites, Inc. (352) 343-3449 has been contacted to confirm any size can be fabricated.

Question: The "Minority and Women Owned Business Enterprises Certification Form" and "Minority and Women Owned Business Enterprises Officer Certification Form" are not in the bid documents, please provide these 2 forms if they are to be part of the package to submit at bid time.
Response: This will be covered under Question 36 of Addendum #6 which will be coming out soon. Copies of these forms will be included in Addendum 6 as well.

Question: With regards to pay item # 5 Demolition, Hauling and Disposal. Certain items will be taken to recycling facilities which may not have the capability to measure incoming loads, how will payment be made for those items without scale tickets?
Response: If contractor elects to recycle any material, contractor will need to provide supporting documentation for the weight of the material recycled. A Public Works Inspector will be assigned to the project and they will need to see all loads prior to transport to the recycler.
