E-Verify forms, E-Verify affidavit, W-9 form, and Disclosure Statement are required for all bidders, not just upon award of the contract. It is requested that the awarded vendor issue their standard agreement, which will be reviewed by Alabama A&M University's Office of the General Counsel.
