Description: The intent of this solicitation and resulting agreement is to obtain the services of a qualified contractor to manage a County-owned group home and a separate attached single-residency occupancy apartment and to provide residential services for individuals with developmental disabilities who may require behavioral and/or medical care and support services. The term of the resultant contract will be one base year with nine one-year option periods.

Question: I am unable to find the form: Contractor Compliance With County COVID-19 Vaccination Policy Certification: included in the RFP document Can you tell me which page of the RFP has the form?
Response: The Contractor Compliance with County COVID-19 Vaccination Policy Certification language in the Proposal Requirements section was incorporated by mistake. There is no certification required for submission or contract performance.
