Question: After reviewing the report that was cited in the RFP, this survey could potentially contain over 1,400 resources. Is this the anticipated number of resources to be surveyed? or is there a different approximate number of resources to be surveyed?

Question: 1. Do Exhibits A and B cover the two required statements (Statement of Compliance with Title VI of the Civil Rights Act of 1964, Section 504 of the Rehabilitation Act of 1973, and the Age Discrimination Act of 1975 and the Debarment Certification) listed on Page 6 of the RFP (Page 7 in the PDF)? The wording on the exhibits doesn’t match the information on Page 6 exactly, so we wanted to verify if we had to include these statements in the proposal language in addition to Exhibits A and B. 2. On Page 7 of the RPF (Page 8 of the PDF), Item 1 indicates that the proposal should be submitted as two separate files, but it does not indicate what information should go in each submittal. Should the proposal portion be one file and the Exhibits a separate file? 3. Item 1 on Page 7 also references Proposal Submittal Contents (items #1-5 on page 8), but Page 8 “Proposal Submittal Contents” only lists four items, and does not specify which sections would go in which of the two submitted documents. 4. Page 11, Chapter 2 indicates that this is a Qualifications-based selection. Just making sure that means that we do not need to submit pricing information with the proposal at this stage (since pricing is referenced on Page 2 of the RFP in the second bulleted guideline under Electronic Submittal Requirements).

Question: 1.) On page 2 of the RFP it states “All requested information and forms MUST be uploaded as one file if possible. If necessary to have more than one upload, pricing, and signed acknowledgments, etc. are to be in the first upload and the Pricing Information should be in the second, with each titled accordingly.” In this statement, “pricing” is listed as going in the first upload and the second upload. Is this correct? Also, on page 7 it states “Electronically submit two separate files to the Procurement Services Director…” Could we be provided with clarification as to how many downloads are required (1 or 2) and if 2 are needed, what information is required in each document, should the proposal be submitted as one Technical Proposal and one Price Proposal? 2.) On page 11 of the RFP, it states in 2-1, “Qualifications-based procedures require that a contract for consulting services be awarded pursuant to a fair and open selection process based on the qualifications of the firms. The fees for such services are established following selection of a firm through a negotiation process to determine a fair and reasonable price.” And in 2-3, “QUALIFICATIONS-BASED SELECTION PROCEDURES. Consultants shall be selected based on their qualifications and experience, with fees determined through negotiations following selection. The highest ranked offeror shall be selected, and price proposals shall be negotiated with one or more of the firms identified through the qualifications-based selection procedure.” Could we be provided with clarification as to if a price proposal is required for this RFP? It is mentioned in other sections, however, this wording makes it seem like a price proposal is not necessary and will be required once a firm is selected based on qualifications, and then a negotiation process will determine the price.

How many resources are expected to be surveyed? We can’t build an accurate budget without this information. 3. Addendum #1 references funding of $40k. Is additional funding available for this project? If so, what is the project’s total budget?
