Question: Section III.B. Submission Proposals Format on page 15 of the RFP lists "Section 6 – Volume of work previously done" as a required section in the proposal. Can you please clarify what information you’d like us to provide for this section?

Evaluation Criteria on page 20 of the RFP says the New Mexico Employees Health Insurance Must be provided (Appendix D); however, this form is not requested in Section III.B. or included in the RFP. Do we need to include this form in the proposal? If so, can you please provide it?
