Description: Proposals to provide the services described herein shall include: a letter of transmittal requesting that the firm be certified as qualified to provide the required professional services, organizational profile and firm’s qualifications, resumes of individuals to perform the work, scope of services proposed, public entity crimes affidavit, standard forms SF 330 and other pertinent data in the information regarding the firm.
• Pavement and shoulder widening, as appropriate;
• Drainage improvements, as appropriate;
• Signing and pavement markings;
• Geotechnical Investigation;
• Pavement Design;
• Traffic Control Design;
• Side Street and Driveway connections;
• Survey and Right-of-Way determination;
• Utility Coordination;
• Stormwater Permitting: Acquisition of all applicable stormwater and environmental permits in accordance with Chapter 62-25, Regulation of Storm water Discharge, Florida Administrative Code; Chapter 373 and 403, Florida Statutes; Chapters 40 and 62, Florida Administrative Code; Rivers and Harbors Act of 1899; Section 404 of the Clean Water Act; and parts 114 and 115, Title 33, Code of Federal Regulations. In addition, permitting required by local agencies shall be prepared in accordance with their specific regulations.; and
