SCOPE OF WORK
The purpose of this bid is to obtain pricing for the following bridge materials for the Poplar Head Church Road Bridge.
BRIDGE STANDARDS – OKALOOSA COUNTY
All construction will be in accordance with standards adopted by the American Association of State Highway & Transportation Officials as set forth in the AASHTO Manual “Standard Specifications for Highway Bridges.”
Bidders are required to complete the blank spaces to the right of each line item showing compliance or listing the exception taken. Either place a check mark or initial to show compliance.
I. CONDITIONS
A) All invoices shall be clearly marked “For Poplar Head Church Road Bridge”.
B) The successful vendor shall guarantee (in writing) their delivery date upon receipt of an order.
C) Each bidder is required to complete the attached bid sheet.
D) All prices must include freight/delivery charges, if any.
E) Delivery will be to: Jobsite. (Map & Directions attached).
F) The successful vendor must notify Okaloosa County a minimum of 24 hours prior to delivery

II. SCOPE OF WORK
A) Design and furnish all materials necessary to construct a bridge with precast or pre-stressed concrete. The total length of the crossing is 96’ long with a total deck width of 24’6”. Items to be included with the bid shall be abutment panels, abutment caps, intermediate caps, wing walls, wing wall caps, bridge decking, bridge deck barrier rails, guard rail and all hardware necessary to construct the bridge. In addition to the total deck length, standard approach slabs, at 10’ long, shall be incorporated in the design.
B) Pilings supplied shall be 50’ in length. For bidding purposes, it shall be assumed that 50 pieces of 50’ length pilings are required. Should additional piling be required to satisfy loading requirements specified in the bridge design drawings, it will be purchased separately from this bid.
C) Must provide design drawings signed and sealed by a Florida Professional Engineer. Drawings shall specify pile capacity requirements to be achieved during construction. The design load shall be HL-93. The design speed of Poplar Head Church Road is 35 mph.
D) Guardrail shall meet the requirements of FDOT Standard Specification 536 and in accordance with FDOT Standard Plans, Index 501.
E) Load rating calculations shall be provided and signed and sealed by a Florida Professional Engineer.
F) All materials shall be delivered to the site and coordinated with the Okaloosa County Bridge Construction crew. Materials delivered damaged or improperly fabricated shall be removed from the site and replaced with proper materials at no additional cost to the County.
TERM
The Agreement will begin once both parties have signed the agreement. A notice to proceed will be issued when the work is ready to begin. The work must be substantially completed with nine (9) months with a full completion date by eleven (11) months.
Insurance Requirements are found in Attachment “B” of the Draft Agreement furnished with this Invitation to Bid (ITB). Respondents should review these requirements prior to submitting a proposal.
